This client registration form is for new pet care clients of HLG Veterinary Physiotherapy — dog walking, drop-in visits, pet sitting and equine care.

This pet care registration form takes about five minutes to complete online. I’m a Masters Qualified, RAMP Registered Veterinary Physiotherapist, and the same professional eye that assesses how animals move comes with me on every dog walk, drop-in visit and pet sitting stay, at no extra cost to you.
