    Client Details

    I confirm that I have downloaded, completed, and sent my Owner Consent Form (PDF or WORD) to HLG Veterinary Physiotherapy directly via email, WhatsApp, or Messenger. See above for forms (black boxes).

    In accordance with the Veterinary Surgeons Act, I may need to get veterinary consent from your vet before we can start physiotherapy with your animal. Do you consent to me contacting them to organise this?

    Do you agree to follow the payment terms and cancellation policy found in my Terms and Conditions? A copy of my Terms and Conditions can be found on my website.

    Would it be okay if I used images and videos of your animal on my social media for educational purposes? HLG Veterinary Physiotherapy will treat these with respect and will ask for permission in each instance to confirm acceptance.
